SFVA/CPCSDA Three Way Scholarship Applications
Step 1: Visit the SFVA Application link below and print and complete the "Three-Way Form".  Student and Parent must sign this form.

Step 2: Complete and attach the following form to the SFVA application.

Step 3: Scan/Email the completed SFVA and CPCSDA forms to CanogaParkSDAed@gmail.com
(Deadline is August 1)

The request to participate in this program must be submitted annually (Due August 1) and approved by the CPCSDA Church Board prior to any church payment being issued to SFVA.
